Sonoma Valley Hospital invites community members to have their medications reviewed for safety in the privacy of their home through an online visit by a hospital pharmacist who can help ensure that there are no potential problems related to drug interactions, dosing, compliance, expiration, and side effects.
The dates are April 1 and April 20.
“These ‘medication check-ups’ will be especially helpful for those with prescriptions from multiple physicians or pharmacies,” said Baqara ‘Maya’ Yoder, PharmD, SVH Medication Review Program Manager.
The program is a virtual version of the Brown Bag medication reviews that SVH and Vintage House offered early last year in which residents could bring their medications into Vintage House in a brown bag for review by a pharmacist.
Appointments can be scheduled from 10:30 am through 3:30 pm; each is 30 minutes long and appointments will be filled on a first-come basis.
“I will give you an updated personalized medication list and relevant information to make sure that you are getting the most out of your medications,” Yoder said.

Yoder, PharmD, is a clinical staff pharmacist at Sonoma Valley Hospital. Previously a retail pharmacist, she completed a certificate in Transitions of Care from Cedars Sinai Medical Center in Los Angeles, and Medication Therapy Management from UC Davis School of Pharmacy.
