Online survey on health, well-being and equity in Sonoma County

The County of Sonoma Department of Health is conducting a community-wide survey to gather perspectives on health, well-being, and equity.

The online survey takes about 10 minutes.

Health Action is a multi-sector collaborative working to implement policy and systems change strategies that foster health equity in Sonoma County. This survey seeks to include your perspectives on what is working well, priority needs and gaps, the ideal future, and the potential role of Health Action in improving health, well-being, and equity in Sonoma County.

All responses are anonymous. Insights will inform the redesign of Health Action.
